Our Practice Offers the Following Service Options:


All yearly memberships can be paid either as a single payment or in monthly installments. We accept checks, cash or credit cards. Members are entitled to an additional 10% discount when a single yearly payment is made up front.

GOLD MEMBERSHIP

Our original membership plan.

  • Unlimited visits, 24/7, in our office, your home or your place of business.
  • Guaranteed same-day or next-day visits.
  • Unlimited access to a physician by phone 24 hours/day (direct access to a physicians’ cell phone after hours).
  • Daily visits if hospitalized – we oversee and coordinate all aspects of your hospitalization.
  • If you go to the Emergency Room, we will meet you there and guide you through the process.
  • Minimum of 3 visits/year (minimum one hour each visit).
  • Primary physician status for all your medical needs, including any referrals you may need for outpatient diagnostic studies, specialists, etc.
  • Annual fee: $1,620.00; if prepaid in a lump sum a 10% discount is applied for an annual fee of 1,458.00. Spouse or additional family members’ fee is $1,020.00; with discount for full payment, $918.00.
  • If paid monthly, $135/month; $85/month each for spouse or each additional household family members.
  • NO additional fees ever for any of our services.

SILVER MEMBERSHIP

An economical version of your Gold Membership – same private, prompt, professional service on a more limited basis.

  • Visits 24/7 access with a maximum of six doctor visits per year in our office. Same day visits not guaranteed, but every effort will be made to schedule visits as soon as possible.
  • Minimum 2 visits per year (minimum one hour each).
  • An additional six phone consultations per year for medical problems or illnesses (24/7 access).
  • We are still your primary care physician for all you medical needs, including referrals.
  • Visits in the hospital or Emergency Room done at your request – each visit constitutes one of your 6 yearly visits.
  • Annual fee: $960.00; if prepaid in a lump sum a 10% discount is applied for an annual fee of $854.00. This fee also applies to spouse or each additional family member.
  • If paid monthly, the fee is $80/month for all members.

FEE FOR SERVICE

Our most flexible option – for those who want to have us available on an “as needed” basis, 24/7. Office visits $150 each; house calls $250 each (all visits are one hour). Additional fee is $250/hour for all services rendered, including phone consultations, discussion of your case with a specialist, reviewing medical records, hospital or Emergency Room visits. Please note that we are not your primary care physician with this option – all referrals must be handled by your current primary care physician.

SPECIAL MEMBERSHIP: RESIDENTS OF EXTENDED CARE FACILITIES

For our members who reside in a nursing home, personal care or assisted living facility.

  • Routine visits are performed according to the facility’s standard.
  • Emergency visits are made when it is deemed necessary by the facility staff.
  • Physicians are available for routine phone falls from patients or their families.
  • We are still the primary care doctor for all your medical needs.
  • Daily visits if hospitalized.
  • Emergency Room visits not guaranteed if the nature of the medical problem requires only “treat and release”.
  • $65/month for all members.

CERTIFICATION FOR FITNESS TO DIVE

Dr. McMicken is a member of the Underwater and Hyperbaric Medical Society (UHMS) and has been qualified to examine both recreational and commercial divers for certification as to fitness to dive. Physical examinations to determine Finess to Dive may be scheduled on a fee for service basis

Dr. McMicken has training as a nuclear trained Submarine and Diving Medical Officer, U. S. Navy, including Submarine School, Nuclear Power Prototype Training and Deep Sea Diving School. He served as Medical Officer of USS Seawolf SSN-575, USS Snook SSN-592, and Submarine Squadron Six. He has additional experience in hyperbaric oxygen therapy.
